Fill out the following 4 page application and remit to the Municipal Office:

  1. Application Form for Membership
  2. Par-Q Form
  3. Emergency Form
  4. The Informed Consent Agreement
*All forms must be complete, signed and dated, leaving no question blank
 
**Memberships and proximity access key tags must be paid for in full, prior to receiving a gym membership.
 
The Municipality reserves the right to refuse a membership based on an incomplete application.

Membership Fees:
18 years or older (Adult) Qualify for a proximity access key tag which gives the member access to the facility 24 hours a day (A $10.00, fully refundable deposit is required to secure a key tag).
Under 18 years do not qualify for a tag and must be supervised while using the facility.

Gym PhotoOur Accomplishments: We are proud to say that our community has become more physically active as a result of having this service in Matachewan. We have organized various activities and events to increase physical fitness such as 12.5 km walk-a-thons, volley ball tournaments, the formations of walking clubs, circuit and stability ball training, yoga and an assortment of exercise programs and have installed an outdoor basketball court.

Gym Equipment: The gym is a heated and air conditioned facility with 2 bathrooms, a conference room and an aerobics/yoga video room. It is equipped with an elliptical trainer, a treadmill, a recumbent bike, a 4 person workout station (leg press, pulley system), free weights ranging from 3-80 lbs, a power cage with Olympic bar and up to 1000 lbs of Eurogrip plates, an arm curl station, and a seated calf raise station. There are also stability balls, exercise tubes/bands, and skipping ropes.

Donations: This facility continues to operate with the help of gym memberships, fundraising and donations. If you are interested in making a financial donation please contact one of the Committee members today.
